In this study, we present our experience with the use of ultrasonography to reduce the use of radiation in closed kidney stone surgery (percutaneous nephrolithotripsy) in paediatric patients. In many parts of the world, a radiation imaging device (scopy) is used to enter the kidney in closed kidney stone surgery. Although this is not much of a problem in adult patients, paediatric patients have high sensitivity to radiation. Therefore, radiation-free (ultrasonographic) or radiation-reduced methods should be used as much as possible, especially in paediatric patients. In this study, we present the successful results of our use of ultrasonography for intraoperative access to the kidney in 17 patients. We are still using ultrasonographic access in closed kidney stone operations especially in paediatric patients.

In this study, we published the results of closed surgery (laparoscopic adrenalectomy) methods that we applied in tumours of the adrenal gland (surrenal gland=adrenal gland), which is involved in the secretion of many hormones of our body and has very critical duties. Removal of the adrenal gland by closed method is a surgical method performed only in certain centres in our country. With this study, our team was the first to perform this surgery by closed method in Diyarbakır province. After this study, a total of nearly 50 patients have been operated on with the closed method and our team has trained many physicians on this subject. Tumour operations of the adrenal gland are more difficult, longer hospitalisation and recovery period is longer when performed by open surgical method. All over the world, almost all open surgeries have been replaced by closed surgery methods, if possible. The most important advantage of closed surgery is that the healing process is very fast since the normal structure of the body tissues is preserved. In closed surgery methods, the operation can be shorter, the patient returns to normal life activities faster, the patient has less scarring, the patient stays in the hospital less, the patient uses less medication after surgery and treatment costs are reduced.

In this study, we described a method we discovered in closed kidney stone surgery (percutaneous nephrolithotripsy) in preschool children. For many years, closed kidney stone surgery in children was performed with instruments used in adults. This situation caused some problems for both the patient and the doctor performing the surgery. With the method we discovered in this study, we described the use of an angiocath (branule, intracath, peripheral venous catheter), which is used in our daily practice to administer intravenous serum. Thanks to this technique and material we have described, we have gained gains such as reduction in the cost of surgery, providing comfort to the physician during surgery, protecting pediatric patients from radiation by using ultrasound, providing an easily accessible material, providing safe access to the kidney in children under 7 years of age, and defining the smallest access needle used in closed kidney stone surgery.

In this study, we present the results of closed renal stone surgery with ultrasonography support in patients with anatomical abnormalities or in patients with disrupted body structure and shape due to musculoskeletal diseases. Thanks to this method, 5 patients with congenital kidney anomalies such as rotation anomaly, 5 patients with horseshoe kidney anomaly, 3 patients with lumbar curvature (scoliosis) and 2 patients with severe lumbar curvature (kyphoscoliosis) were successfully removed from the stones in their kidneys by closed kidney surgery (percutaneous nephrolithotripsy). Patients with this deformity cannot be intervened in many centres due to lack of sufficient experience. With the method we described in this study, we have provided a new approach in the intervention of such patients. After the publication of this article, many patients with such deformities have been intervened with the same method. We have become a reference centre for the intervention of these patients in our country.

In this study, we have described a method that facilitates Microperc (Microscopic percutaneous nephrolithotripsy=Microscopic closed kidney stone surgery), which has been used in kidney stone surgery especially in children in recent years. A standard closed kidney stone surgery is performed by creating a tunnel of approximately 10mm (1cm) wide from the skin on the back into the kidney. In this case, we encounter some undesirable results such as bleeding, especially in children. For this reason, the instruments used in this surgery have been miniaturised. Finally, thanks to an instrument (microperc) with the thickness of a needle (4.8f=1.6mm), these surgeries have been performed with fewer complications. We have defined a material and method that will significantly facilitate the use of this device in closed kidney stone surgery, especially in children. Our method has been praised by the autonomists who introduced the microperc device to the literature. By using this method, we have performed closed kidney stone surgery more comfortably in hundreds of paediatric patients so far.

In recent years, the use of robots in the field of health and surgery has increased. The use of “da Vinci robotic system” has become popular especially in some urological cancer surgeries. In a research we conducted, we found that the use of robots in the field of health was first in the province of Diyarbakır, contrary to what is known. We have seen that the scientist named “El-Cezeri”, who lived in Diyarbakır between 1136-1206, designed robotic systems that enable the measurement of blood taken from patients. In this article, we have gained a different perspective to the literature by mentioning these robots and their usage areas.

In a previous study, we described microscopic closed renal surgery (Microscopic percutaneous nephrolithotripsy=Micropercutaneous nephrolithotripsy=Micropercut) by entering the kidney with an angiocath (branula, intracath, peripheral venous catheter) which is used to administer intravenous serum in our daily practice. In this study, we also described a drainage technique in order to make the operation more comfortable and to ensure patient safety in cases that sometimes take too long. Thus, we have seen that we can safely perform microscopic closed kidney surgery in very young paediatric patients with large stones, even if the operation takes a long time, thanks to this technique.

In this study, we presented the results of our successful closed kidney stone surgery in a patient whose body shape was very distorted due to severe curvature of the lumbar bones. In a normal patient, closed kidney stone surgery is performed with the patient lying prone. However, what makes this operation different is that although the patient could not be given the standard prone position, this operation could be performed with the help of ultrasonography.

In this study, we published the results of the first 100 laparoscopic closed surgeries that we were instrumental in starting in Diyarbakır province and in which I took part in the team. Thirty four laparoscopic simple nephrectomy, 22 laparoscopic radical nephrectomy, 21 laparoscopic renal cyst excision, 7 laparoscopic orchiectomy, 4 laparoscopic ureterolithotomy, 4 laparoscopic adrenalectomy, Laparoscopic orchiopexy was performed in 3, laparoscopic pyeloplasty in 2, laparoscopic nephroureterectomy and hand assisted cystectomy in 1, laparoscopic nephroureterectomy and hand assisted partial cystectomy in 1, laparoscopic nephroureterectomy in 1. The number of these operations has approached 500 as of now.
